The Form trigger — When someone fills in a form — builds a simple web page with your questions. Share its link; each time someone fills it in and selects Send, a run starts with their answers. No one needs a Bizomate account to fill it in.

Who can do this

Workspace Admins and Editors, on every plan. A logo on the form needs a paid plan.

Steps

  1. Open the workflow in the designer.

  2. On an empty workflow, select A form is filled in. On one with a trigger, select it, then Change how it starts, then Form.

    The canvas gives way to Form preview — the form as a visitor will see it, updating as you edit: Updates as you edit. Nothing is sent from here. Switch between desktop and phone width with the two buttons. Select Canvas to see the workflow again.

  3. In Form title, type the heading — for example Ask us about an order.

  4. Optional: in Under the title, add one or two lines. Leave it empty and the form starts with its first field.

  5. Set up the Fields. A new form starts with Your name and Email, both required. See Set up the fields on a form.

  6. Optional, on paid plans: under Logo, select Upload a logo and choose a PNG, JPG or SVG up to 1 MB. Shown above the title.

  7. Optional: in After it is sent, type the message people see once they have sent it. Left empty, they see Thank you. Your answers have been sent.

  8. Under Address, see the link the form will have — Made from the title. Not live yet — the form opens when the workflow is published.

  9. Add the steps that use the answers, test with Test run, then Publish.

Once published, Address shows the live link with Copy and Open. Share it however you like.

What a visitor sees

  • The title, the lines under it, your logo, and the fields. Optional fields say · optional; required ones say nothing.
  • A human check before Send, to keep out automated sending.
  • Send reads Sending… until the answers are in.
  • Then Sent and your message.
  • At the foot, Powered by Bizomate.

What happens next

  • Each send starts one run of the live version, in Production, and counts in the month's allowance.
  • The answers are the run's first item, by each field's key: {{ trigger.email }}, {{ $json.your_name }}, and {{ trigger.submittedAt }} for when it was sent. Numbers come as numbers and dates as 2026-10-05.
  • A file someone attaches travels with the item as a file, kept with the run in private storage. See Pass files between steps.

Change the address

  1. Select the Form trigger. Under Address, select Change the address.
  2. Type the new last part — Lowercase letters, numbers and hyphens, 3 to 60.
  3. Select Save address.

A toast says Address changed — The old address keeps working until another form takes it. Changing the title does not change the address.

Good to know

  • The form opens only while the workflow is published and switched on. Otherwise visitors see This form is not taking answers right now — Try again later, or get in touch with whoever sent you the link.
  • A deleted workflow's form shows We cannot find that page.
  • A test run fills every field with a sample answer — Asha Raghunathan, asha@northwind.example — and no file.
  • On Free, the form shows no logo: Your brand logo on the form is on paid plans. On Free the form shows only its fields, with no logo.
  • A send can carry up to about 110 MB in all; each file up to 10 MB.

If something goes wrong

In the designer:

Give the form a title. Form title is empty. Type a title.
That file is 1.4 MB. The limit is 1 MB. The logo is too large. Use a smaller file.
That is a .gif file. Use a PNG, JPG or SVG. / That file is not really a .png. Use a PNG, JPG or SVG. The logo is not a PNG, JPG or SVG. Save it as one of those.
That SVG has a script in it, so it cannot be used. Export it again without one, or use a PNG. The SVG contains a script. Export it again without one.
The address can be changed once the workflow is published. The form is not live yet. Publish first.
Another form in Consulace already uses this address — “…”. The address is taken in this workspace. Select the suggestion after Try, or choose another.
